🏃 SCHOOL SPORTS PHYSICALS — PLEASE READ!

If your child plans to participate in a school sport, there are TWO different requirements you need to understand.
⚠️ PLEASE NOTE: These requirements are STATE LAW and are established by the State of New Jersey. They are NOT Memorial School rules or requirements created by Memorial staff.
🔎 You can check your child's current physical status in the Genesis Parent Portal.

🩺 1. SPORTS PHYSICAL

A sports physical is valid for 365 DAYS from the date of the physical.
EXAMPLE: Your child has a physical on September 1, 2026. The physical is valid through August 31, 2027.
✅ Physical = good for 365 days
❌ It does NOT automatically last for the entire school year.
👉 Your doctor completes the physical packet.

📋 2. HEALTH HISTORY UPDATE FORM

This is NOT another physical. This form is completed by the PARENT/GUARDIAN when the student's most recent physical was completed more than 90 days before the start of the sport.
EXAMPLE: Your child had a physical on September 1, 2026 and wants to participate in a sport beginning after November 30, 2026.
➡️ It has been more than 90 days since the physical, so you must complete a Health History Update Form.
If your child then wants to participate in another sport beginning after March 1, 2027:
➡️ Another 90+ days have passed, so you must complete ANOTHER Health History Update Form.
👉 The Parent/Guardian completes the Health History Update Form.

⏰ DON'T WAIT UNTIL THE LAST MINUTE!

If you know your child's physical will expire before an upcoming sports season, schedule the appointment EARLY. Doctor appointments can fill up quickly, and waiting until tryouts are about to begin does not change the State requirements.
⚠️ Some insurance plans may only cover one routine physical within a certain time period. Check with your insurance company if you are unsure about your coverage.
If your insurance will not cover another physical yet, you do not necessarily have to wait for your child's regular doctor. Sports physicals may also be available for a relatively low out-of-pocket cost at walk-in clinics, urgent care centers, MinuteClinic-type locations, and other providers.
👉 Call the location first to confirm that they perform sports physicals and can complete the required New Jersey school sports physical paperwork.
🚨 Please plan ahead. A student cannot participate until all required paperwork is completed and approved.

⭐ THE REALLY EASY VERSION

🩺 PHYSICAL = Valid for 365 days. Doctor completes it.
📋 HEALTH HISTORY UPDATE = Required when the physical is more than 90 days old at the start of a new sport. Parent/Guardian completes it.
🚨 PHYSICAL EXPIRED = A new physical is required.
🔎 NOT SURE? = Check your child's physical status in Genesis.
⏰ EXPIRING SOON? = Make the appointment NOW, not the day before tryouts.

BEFORE YOUR CHILD SIGNS UP FOR A SPORT:

☐ Check Genesis for your child's current physical status.
☐ Is the physical still within 365 days?
☐ Will the physical be more than 90 days old when the sport begins?
☐ If YES, did I submit a Health History Update Form?
☐ Has the physical expired? If YES, a new physical completed by a healthcare provider is required.
☐ Is the physical going to expire soon? Schedule an appointment now.
⚠️ Submitting paperwork for a previous sport does not necessarily mean your child is cleared for the next sport.

REMEMBER:

These are State of New Jersey requirements. Memorial School must follow the state regulations and cannot waive these requirements.
