(The sign off sheet is due to the health office prior to the first official practice session of the athletic season and annually thereafter).

(Example, if your child got a physical on September 1, 2016, the physical is good until August 31, 2017 - Physicals are valid for 365 days).
Click link for the Health History Update for Athletic Participation Form
(Example, if your child got a physical on September 1, 2016 and wants to play a sport/intramural on or after November 30th (90 days from the date of the physical) the parent must fill out a Health History Update Form and if the child wants to play another sport/intramural on or after March 1st, the parent must fill out ANOTHER Health History Update Form (because it has been another 90 days since the last form was turned in.)

Student Athlete Forms
The following documents are to be distributed annually to every student-athlete and cheerleader in any grade level, prior to participation in an athletic activity, in each school district and nonpublic school that participates in an interscholastic sports, intramural sports, or cheerleading program:
- Student-Athlete Cardiac Assessment Professional Development Module

Important Note: The Cardiac Assessment Module is to be completed by all healthcare providers prior to any student-athlete physical examination.
