❗ Tryouts for the 2021 Fall season will take place on 9/12, 9/13 and
9/14.
🕖 Before Tryouts:
Please submit all pertinent paperwork to Memorial office by 9/7. All forms are available on the Memorial nurse's page: https://memorial.medford.k12.nj.us/apps/pages/index.jsp?uREC_ID=1754680&type=d&pREC_ID=1935650.
Use the following checklist to help you navigate the three categories on the left of the page. Certain forms may or may not apply to you (e.g., the Asthma Treatment Plan), but remember to submit all that do. The following are mandatory for every player:
- Participation in Sports Forms
✅ Physical Form (or Health History Update, if you've had a recent physical)
✅ Opioid Fact Sheet
- Concussion Forms
✅ Concussion Acknowledgement
- Health and Information
✅ Administration of Medication
- Continue on Rt.70E crossing over Rt.73.
- Turn RIGHT onto Old Marlton Pike, also called Union Road.
- Pass through the light at Hartford Road.
- Approximately ¼ mile Freedom Park entrance will be on the Right.
- Do Not follow road to the back, proceed to the left as if you going to exit the park.
- Turn Right on a dirt driveway looking road, it will open into a parking lot. Field will be in front of you.
- Take Rt.70W turn LEFT onto Jones Road (After Dunkin Donuts)
- At the stop sign/end of the road, Freedom Park will be diagonally across the street.
- Turn right onto Union Road/Old Marlton Pike then left into the park.
- Do Not follow road to the back, proceed to the left as if you are going to exit the park.
- Turn Right on a dirt driveway- looking road, it will open into a parking lot. Field will be in front of you.
